The Held In Our Hearts team are so excited to share that we are hiring! ? We are recruiting for 2 part time Hospital to Home Workers, as part of a 3 year Big Lottery funded project. The pilot project, Hospital to Home Care, will support families to transition from the hospital setting to being back in the community.
