Hospital to Home

Hospital to Home (H2H) is a project in partnership with the NHS that supports parents whose baby has died after 12 weeks of gestation and up to pre-school age.

This bespoke service for families aims to support the transition from hospital to home, helping to ensure that no family falls through the gap and goes home without bereavement support when they leave the hospital. Hospital to Home is currently available within NHS Lothian, NHS Fife, NHS Forth Valley and NHS Highland.

Referrals for this service can be made by any health professional within the first 3 weeks after loss. All referrals will be handled in the strictest confidence and in line with GDPR.
