Make a referral for Hospital to Home support:

Hospital to Home (H2H) is an innovative project specifically focusing on early intervention for pregnancy, baby and infant loss for any family who has experienced a loss after 12 weeks.

(Please note, referrals from NHS Lothian are from 12 – 24 weeks only at this time.)

This bespoke service for families aims to support the transition from hospital to the home, helping to ensure that no family falls through the gap and goes home without bereavement support when they leave the hospital.

Referrals for this service can be made by any health professional within the first 3 weeks after loss. All referrals will be handled in the strictest confidence and in line with GDPR regulations.
