Providing early intervention for families in the immediate days and weeks following the loss of a baby

Hospital to Home

This is a pilot project in partnership with NHS Lothian and is available for families in this Health Board.

Hospital to Home is the first of its kind in the UK specific for baby loss. The project is part funded by the Big Lottery. The aim is to support families whose baby has died between 14 and 24 weeks in pregnancy.

The project intends to fill a gap in the transfer of bereaved families between hospital and home by providing bespoke and targeted short term support to bereaved families after the devastating loss of their baby.

This allows the H2H support workers to reach in so families do not have to reach out for support.
